Taking Steps Today to Support Tomorrow’s Fertility
Wondering when to seek help for fertility concerns? Dr Kasturi Moodley explains why an early fertility assessment can provide answers, reassurance and a personalised plan for your journey.
Introduction
Trying to conceive can be an exciting and deeply personal experience. But when pregnancy doesn’t happen as quickly as expected, it can also bring uncertainty, questions and anxiety.
One of the most common questions couples ask is:
“When should we seek help?”
Many couples continue trying because they aren’t sure when it is appropriate to speak to a fertility specialist. Others worry that making an appointment means something must be wrong or that they will immediately be expected to start treatment.
An early fertility assessment does not necessarily mean that there is a problem, nor does it mean that treatment is required.
Instead, it can provide an opportunity to understand your individual fertility, identify any potential concerns and discuss the most appropriate next steps for you.
What Is a Fertility Assessment?
A fertility assessment is an opportunity to look at the factors that may influence your ability to conceive.
Every couple is different, so the assessment will be tailored to your individual medical history, age, circumstances and fertility goals.
Depending on your situation, an assessment may include:
- Medical and reproductive history – including previous pregnancies, medical conditions, surgeries and medications.
- Lifestyle assessment – discussing factors such as nutrition, exercise, smoking, alcohol, weight and general wellbeing.
- Ovulation assessment – determining whether ovulation is occurring regularly.
- Hormonal testing – blood tests may be recommended to assess particular aspects of reproductive health.
- Ultrasound examination – which may provide information about the uterus and ovaries.
- Semen analysis – assessing sperm health is an important part of evaluating male fertility.
- Individualised recommendations – based on the results of your assessment and your personal circumstances.
Not everyone will require every test. The aim is to ensure that investigations are appropriate to the individual rather than following a one-size-fits-all approach.
When Should You See a Fertility Specialist?
There is no single timeframe that applies to every couple.
For couples without a known fertility concern, general guidance can provide a useful starting point.
If You Are Under 35
If you are under 35 and have been having regular, unprotected intercourse for 12 months without achieving pregnancy, it is generally recommended that you seek a fertility evaluation.
If You Are 35 or Older
If you are 35 or older, it is generally recommended to seek professional advice after 6 months of trying without achieving pregnancy.
As fertility can change with age, seeking advice sooner can be particularly valuable as you get older.
If You Have Known or Suspected Fertility Concerns
You don’t necessarily need to wait six or twelve months if there are already reasons to be concerned about fertility.
Earlier assessment may be appropriate if you have conditions or circumstances such as:
- Polycystic ovary syndrome (PCOS)
- Endometriosis
- Irregular or absent periods
- A history of recurrent miscarriage
- Previous pelvic or reproductive surgery
- Known or suspected male fertility concerns
- Previous fertility problems
If you have concerns about your fertility, discussing them with your doctor can help determine when an assessment would be appropriate for you.
Why Can Early Fertility Assessment Be Important?
One of the biggest benefits of an early assessment is clarity.
Rather than spending months wondering whether something might be affecting your fertility, an assessment can help provide a clearer understanding of your individual circumstances.
Depending on your situation, early assessment may provide:
Earlier Answers
Identifying potential fertility factors can help you understand why conception may be taking longer than expected.
Reduced Uncertainty
Having information about your fertility can replace some of the worry and uncertainty with a clearer picture of your situation.
More Time to Consider Your Options
Understanding your fertility earlier may give you more time to consider different approaches and make informed decisions.
Lifestyle Guidance
Your consultation may identify lifestyle or health factors that could be addressed as part of your overall fertility care.
Personalised Care
Rather than relying on general advice or comparing your journey with someone else’s, you can receive guidance based on your individual circumstances.
Better Planning
Understanding your fertility can help you and your healthcare team determine the most appropriate next steps.
Fertility Is a Shared Journey
When a couple experiences difficulty conceiving, the focus can sometimes fall primarily on the woman.
However, fertility is a shared journey.
Male fertility factors can contribute to difficulties conceiving, which is why assessing both partners can be an important part of the fertility evaluation.
A male fertility assessment may include a medical and reproductive history and a semen analysis, with further investigation recommended where appropriate.
Taking a couple-based approach can help ensure that both partners are considered and supported.
Most importantly, fertility difficulties are not about blame.
They are about understanding what may be contributing to the difficulty and determining what support may be appropriate.
What Happens After a Fertility Assessment?
There isn’t one treatment pathway that is right for everyone.
Depending on the findings of your assessment, the next step may simply be continued attempts to conceive with appropriate guidance.
For others, further investigation, medical management or fertility treatment may be recommended.
The important thing is that your care should be based on your individual circumstances.
Seeing a fertility specialist does not automatically mean that you will need IVF or other fertility treatment.
Sometimes the most valuable outcome of an assessment is simply having your questions answered and understanding your options.

It can provide an opportunity to ask questions, explore your options and determine whether further investigation or support is appropriate.
